The undersigned being the lawful parent and/or guardian of the listed child(ren) hereby consent to the participation by the child(ren) in all activities conducted by The Hive- Northlake and to the participation of the child(ren) in all events related to said activities.


The undersigned assumes all risk of injury or harm to the child(ren) associated with participation in The Hive-Northlake’s activities and agrees to waive, release, indemnify, defend, hold harmless, and forever discharge The Hive-Northlake and its managers, members, contractors, assigns, guests, business invitees, subsidiaries, vendors, staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of accident, death, injury, loss or damage to the child(ren), and the child(ren)’s property, howsoever caused, arising by reason of or during the child(ren)’s participation in The Hive-Northlake.

I fully understand that these activities MAY involve risks of bodily injury resulting from use of equipment and other obvious injuries that may result from participation. And, furthermore, I assume all risk of injury which may be caused by my child(ren)’s own actions, or inactions, those of others participating in the activities, the conditions in which the event take place; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs and damages my child(ren)’s incur as a result of my participation of the activity. I acknowledge and understand that included within the scope of this waiver and release is any claim or cause of action arising from the performance, or failure to perform, of any maintenance, inspection, supervision control of said premises and for the failure to warn of dangerous conditions existing at said premises, for the failure to maintain or inspect equipment supplied to my child(ren), for negligent selection or retention of certain releases, or negligent supervision or instruction of personnel. I understand and agree that all the risks and dangers described throughout this agreement, including those caused by my child(ren)’s negligence and/or others, are included within the waiver and release contained herein. I (and as parent(s) or guardian(s) of a Participant who is a minor, if applicable), KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN.

I hereby certify that my child(ren) do(es) not suffer from any physical infirmities, chronic illness or physical disabilities which are affected by or prohibit my child(ren) from engaging and participating in activities. I also understand that if my child(ren) do(es) suffer from any physical infirmities, chronic illness or physical disabilities I understand that I am responsible for consulting my child(ren)’s physician before my child(ren) engage(s) in any activity that may aggravate any condition that my child(ren) may have. I also understand that if any physical or mental conditions exist that I must notify The Hive-Northlake in advance of any event.
